I grant permission to DSEC/Dinamite Volleyball Club, its representatives, and employees the right to take photographs and videos of my daughter during her participation in activities with DSEC/Dinamite Volleyball Club.

I authorize DSEC/Dinamite Volleyball Club, its assigns, and transferees to copyright, use, and publish the same in print and/or electronically. I agree that DSEC/Dinamite Volleyball Club may use such photographs and videos of me with or without my name and for any lawful purpose, including but not limited to publicity, illustration, advertising, and web content.

I hereby waive any right to inspect or approve the finished photographs or videos or any printed or electronic matter that may be used in conjunction with them, now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photographs or videos.

Release of Claims: I release DSEC/Dinamite Volleyball Club from any claims, demands, actions, causes of action, or suits arising out of the use of such photographs and/or videos, including but not limited to any claims for defamation, invasion of privacy, or infringement of moral rights.

No Compensation: I understand and agree that my dependent or I will not receive any compensation for the use of these photographs and/or videos.

No Obligation: I acknowledge that DSEC/Dinamite Volleyball Club is under no obligation to use the photographs and/or videos taken of my dependent.

 

I HAVE READ THIS AGREEMENT AND UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A COMPLETE RELEASE OF LIABILITY AND A BINDING CONTRACT, AND I SIGN IT OF MY OWN FREE WILL. I CERTIFY THAT I AM AT LEAST 18 YEARS OF AGE ON THE DATE HEREOF, AND IF I AM SIGNING ON BEHALF OF A MINOR PARTICIPANT, THAT I AM THE PARTICIPANT’S PARENT OR LEGAL GUARDIAN.
