The release, waiver, and assumption of risk form for Dinamite Volleyball Club (DVC) activities in 2024 requires parents/guardians to acknowledge the inherent risks of participation, including potential injury, illness, or death. Additionally, it includes a release for using the participant's likeness and a specific waiver related to COVID-19, stating that DVC cannot guarantee protection against the virus and that participants assume the related risks.
